Vasool Raja, is the leader of a rowdy gang which uses violence to illegally extort money from people. To keep his parents oblivious to his real profession, he employs an extravagant illusion, pretending to be a medical doctor running a hospital. However, when his parents attempt to arrange his marriage with an old friend's daughter, the truth is starkly revealed. Ashamed and willing to do anything to win back his parents' trust, Vasool decides to attend medical school to earn a real medical degree. There he encounters his first love (childhood friend) and also his arch-enemy. How will Vasool Raja deal with the rigors of medical school? How will the doctors, students and patients be influenced by his fresh and empathetic perspective to healing?

Saran hoped to sign on director K Balachandar to play Kamal Haasan's father in the film, with the veteran director being both Kamal Haasan's and Saran's film industry mentor. However Balachandar was reluctant to act and the team finalised Girish Karnad for the role, before later replacing him with veteran actor Nagesh.

Sonali Bendre, who was working on the Telugu version of the same film 'Shankar Dada MBBS', was initially approached to play the leading female role but her reluctance to accept the offer immediately meant that the team sought and consequently signed on actress Sneha for the film.

The film faced controversy as the petition filed by Tamil Nadu Medical Council president K R Balasubramanian stated that the film's title ridiculed the medical profession and tarnished the image of the medical fraternity. Moreover, the film title is a mockery on the medical profession which would lower dignity of the medical fraternity in eyes of public.

It was the first Kamal film to beat the record set by 'Indian' eight years earlier.

This film is a remake of the phenomenally successful Hindi film 'Munnabhai MBBS'.
