Embedded with mystery and suspense, this paranormal thriller depicts the adventures and exploits of renowned Psychiatrist Dr. Sunny Joseph, who lives in America. He decides to travel to a town in India after he gets a distress call from his friend, who belongs to a very noble and wealthy family, to counsel and treat a female family member, who he suspects is responsible for a variety of incidents. Sunny accepts this assignment not realizing that there is another side to this tale, namely a vengeful spirit, who may not respond to his methods, and he may end up endangering everyone's lives.

Trivia:
Many dubbing artists were used in this movie like, Bhagyalakshmi for Shobana's Ganga portion, Nagavally's portion by Durga, a famous Tamil dubbing artist. There is strong opinion that both Ganga's and Nagavally's portion were dubbed by Bhagyalakshmi. The other dubbing artists work: Anandavally for Vinaya Prasad and Ambili for Rudra (Alli).

Major parts of the film and the climax scenes were filmed in Padmanabhapuram Palace and Hill Palace, Thripunithura.

'Geethaanjali', a spin-off directed by Priyadarshan and Mohanlal reprising the role of Dr. Sunny Joseph, was released on November 14, 2013.

This movie was remade after nearly 10 years in various languages, including Kannada (Apthamitra), Tamil and Telugu (dubbed) (Chandramukhi), Bengali (Rajmohol), and Hindi (Bhool Bhulaiyaa), all being commercially successful.

After becoming phenomenally successful upon release, its popularity skyrocketed to the extent that it is now considered to be one of the best thrillers ever made in India as well as one of the best Malayalam films ever made.

When the movie was announced several filmmakers suggested director Fazil to cast Mammootty for the part of psychiatrist "Dr. Sunny Joseph", but he wanted the character to be light hearted and funny, so he cast Mohanlal instead.
